After the huge success of my All About My Mom printable, I thought it would be fun to create a Printable Mother’s Day Questionnaire.
As you can tell, we’ve always loved filling in questionnaires. It makes the kids so happy to make a special gift on their own, and you can keep them to remember all their sweet answers once they are grown. (And to have a good laugh at their crazy occurrences).
I designed these Mother’s Day questionnaires to be printer-friendly, making them perfect for classroom use or working with a large group of kids.
And they have some cute little drawings on the sides that can be colored and decorated to make these pages even more unique.
Then, your kids can continue to fill in the answers on their own. And if they are too young to write, you can ask them the questions and write down their answers for them.
Super simple, right? And a great way to work on critical thinking and even improve writing skills.

In this printable, your kids will have to write down fun facts about mom, including:

- Mom’s favorite thing to do
- Mom’s favorite food
- Mom’s favorite thing to watch
- An activity mom is good at
- What princess she would be
- What she would do with a million dollars
- And finally, a reason why mom loves them

Grandma Questionnaire
After getting many requests, I also had to make a grandma questionnaire!
It has the same design and questions as the mom’s version, so your kids won’t miss any fun facts about grandma.
I’m sure you can’t find an easier Mother’s Day activity to celebrate Mom and Nana!
How to Download the Mother’s Day Questionnaire Printable
All you have to do to download the Mother’s Day questionnaires is click the red link below.
You’ll get both versions, so print the ones you need on regular white paper. Remember to select Fit Page on your printer settings for best results.
Then, let your kids color their page and answer the questions however they want. Remind them that there are no wrong answers and just write the first thing that comes to mind.
Once they have completed their questionnaires, you can laminate them or gift them on their own.
Moms and grandmas can even keep them in a safe binder to look over them with their kids when they’re older.
